When we called the toll free number the automated voice could not understand or decipher either spellings or numbers. Very frustrating. Also, would have liked to have printed the form to review it, but it was not accessible. Further, the form generated on screen was small print and was in light blue lettering which gave me a headache, plus I'm partially color blind (red green) but the very light blue lettering was difficult for me to see.
