I converted a traditional IRA to a Roth IRA. I am with TD Ameritrade and the question is who is on the form it asks who its paid by...is it me or TD Ameritade?

Probably TD Ameritrade. What is on the Form 1099-R you received? TD Ameritrade should be the payer since they issued the Form 1099-R.
