What does a code G mean in box 7 on a 1099-R?

My 401 K which had stayed at the same investment firm for 4 years after I left the company I worked for was closed and moved by my former employer to a second investment company without them contacting me first. I took a total distribution of my funds from the second investment company. I received a 1099R from the first investment firm and another 1099R from the second investment firm it was moved to, do I have to input both of them or just the later one?

Code G means rollover
You must report all 1099-Rs you received. The IRS gets copies of the and will expect to see all of them reported.
