I cashed in US Savings bonds. Do I enter the bank name or "US Savings Bonds" in the received from line?

If you cashed them in at a bank, use the bank name as the "received from". When you enter the interest on form 1099-INT, put the savings bond interest in box 3 (US Treasury interest). That way, you will only play federal tax on it, not state tax.
