IRS Letter Says They Didn't Receive Form 8453-OL

The IRS requires a signature even when you e-file your tax return.

If you e-filed your return and did not submit an electronic signature, then you must submit a signature on paper using "Form 8453-OL, U.S. Individual Income Tax Declaration for an IRS e-file Online Return." To learn all about Form 8453-OL, see Working with Form 8453-OL.

If you received a letter from the IRS indicating that they did not receive Form 8453-OL, then you need to fill out Form 8453-OL and mail it to the IRS in order to complete your tax return.

How to complete Form 8453-OL
  1. Print a copy of Form 8453-OL using the TurboTax program.
  2. Verify that your Declaration Control Number (DCN) printed on Form 8453-OL. The DCN is the confirmation number that is assigned when your return is accepted and must be included on Form 8453-OL.

    If the DCN did not print on your paperwork, you'll need to enter it yourself. Here's how:
    • Check the status of your return in the TurboTax program and copy the DCN from the acceptance message.
    • Ignore the first two digits ("00") in the first box of the DCN and then write the next six digits of your DCN in the second box in the upper-left corner of Form 8453-OL.
    • Write the next five digits in the third box.
    • Ignore the last digit in the fourth box of the DCN.
  3. Fill out the form including your name, address, social security number, tax return information, and signature. If you filed jointly, include your spouse's information and signature as well.
    • To include tax return information, refer to a copy of your tax return. Or, you can call the IRS at 1-800-829-1040 to request your tax return information.
  4. Attach any required documents that were not included in the e-filed return (such as "Form 8885, Health Coverage Tax Credit").
  5. Mail Form 8453-OL (including attachments) to the IRS to the address listed for your state.
