Utah Residents Form 1099-R (1099-R Issuer's name) State 1 payer number must end with WTH Error

Utah changed the format of their Employer’s State ID Number from seven to fourteen characters.
The correct formats are:
- 11 digits followed by ‘WTH’ for W-2's and 1099's.
- 11 digits followed by ‘WMP’ for Mineral Production.
Solution:
If prior year information was transferred into a 2011 return with the old 7 character format for the Employer State ID.
Then replace the old 7 character Employer ID number in Box 13 of the 1099 with the Employer's new 14 character State ID number from the 1099.
If the 1099-R contains the issuers’s old State ID number.
Then you must get the new State Employer ID Number from the issuer of the 1099-R.
