Form 1040-V: Payment Voucher is an IRS form that you include with your check or money order for any balance due on the "Amount You Owe" line of your federal tax return.
Although Form 1040-V is optional, it does help the IRS to process your tax payment more accurately and efficiently.
TurboTax will only create Form 1040-V if you owe federal taxes on your return. If you are getting a federal refund, this form will not be generated because it is not needed.

TurboTax Online
If you file by mail and choose to pay your taxes via check or money order, TurboTax Online will automatically print Form 1040-V along with the other forms in your tax return.
If you e-filed and want to print Form 1040-V:
- Sign into TurboTax Online. If you're already signed in, click the Home tab.
- On the Welcome Back screen, choose Print My Documents.
- Follow the on-screen instructions to print any required paperwork, which will include the payment voucher.
- Mail your payment and the voucher to the address on the voucher.
TurboTax CD/Download software
If you file by mail and choose to pay your taxes via check or money order, TurboTax will automatically print Form 1040-V along with the other forms in your tax return.
You can also print Form 1040-V separately by following these instructions:
Windows
- Open your tax return.
- Click the Print Center icon in the upper right corner.
- In the Print window, select Specific Forms.
- On the right side of the Print window, select Form 1040-V: Payment Voucher (you may need to scroll down to see it).
- Click the Print button.
- Mail your payment and the voucher to the address on the voucher.
Macintosh
- Open your tax return.
- From the TurboTax File menu, choose Print.
- Select Print Selected Forms from the drop-down menu at the top.
- Select Form 1040-V: Payment Voucher (you may need to scroll down to see it) and click Continue.
- Click the Print button.
- Mail your payment and the voucher to the address on the voucher
