Form 1040-V is a voucher you send with your check or money order for any balance due on the Amount you owe line of your 2012 Form 1040, Form 1040A, or Form 1040EZ.

Who needs to file a form 1040-V?

Only those taxpayers who owe a payment to the IRS on their return, and are mailing in their payment, need to file form 1040-V.
